I would like to recommend the use of a medical information form developed by Joyce Cocchi. Her mother is a patient under my care. Ms. Cocchi has used her medical information form that includes her current medications to keep me abreast of the patient’s present use of medications, as well as other pertinent information. This I would find quite useful for any person, particularly those dealing with more than one physician, or on entering a medical facility.
